If you are planning to return to work during the 2017 – 2018 school year, you MUST:
1. Complete the “Substitute Status” form located in the Employee Self Serve section of the DDSB Staff Portal no later than June 30th, 2017. (This does NOT apply to Emergency Supply Teachers)
If you are on more than one supply list (i.e., Supply Teacher and Casual EA), you will be required to fill out a Substitute Status Form for each list that you belong to.
However, if you are a Supply Teacher that works on both the Elementary and Secondary panels, you will only be required to fill out one form. Should you require changes to your name, phone number, qualifications, etc., please send an email to: email@example.com with the changes
Important: If you are on an approved leave, you must still complete the substitute status form if you wish to remain on the Roster and return to work as an OT in the future.
2.Complete the electronic Offence Declaration for the upcoming school year 2017 – 2018, which will soon be uploaded and found in the Employee Self Serve section, (same location as the Substitute Status Form) usually no later than September 15th of the new school year.
3. Ensure the Local has your current home address. The Local’s annual mailing of: a Day Planner to track your assignments and provide you important information at your fingertips; a copy of the extension agreement 2017 – 2019 to the 2014-2017 Collective Agreement; Understanding your Professional Judgement information; Local Newsletter; PL information; and more will be sent mid-August. Every year, approximately 30 – 40 packages are returned to the Local. Please notify the Local office if you have moved within the past year by emailing firstname.lastname@example.org.
