Urgent Reminder! Substitute Status Form Due June 30, 2016

f you are planning to return to work during the 2016 – 2017 school year, you must complete your Substitute Status Form by June 30th!
 
There are approximately 400 OTs who have yet to complete their form.
 
  • If you have applied for a permanent job, do NOT wait, complete your substitute status form now.
  • If you are unsure whether you completed your form, follow the instructions below and check the “completed forms” section to make sure it is there.

This is a reminder, at the end of every school year, if you wish to return to Occasional Teaching in September, you MUST: 

1. Complete the “Substitute Status” form located in the Employee Self Serve section of the DDSB Staff Portal no later than June 30th.  
 
If you are on more than one supply list (i.e., Supply Teacher and Casual EA), you will be required to fill out a Substitute Status Form for each list that you belong to.
 
However, if you are a Supply Teacher that works on both the Elementary and Secondary panels, you will only be required to fill out one form.   Should you require changes to your name, phone number, qualifications, etc., please send an email to: sfe.support@ddsb.ca with the changes  
 
2.Complete the electronic Offence Declaration for the upcoming school year (2016 – 2017)  found in the Employee Self Serve section, (same location as the Substitute Status Form) usually no later than September 15, 2016.
 
To access these forms, click on the Employee Portal on the home page of the DDSB website. Enter your ID (include @ddsb.ca at the end). Once you are in the Portal, Click on “Quick Links” and scroll down to the Employee Self Serve (ESS). This is where your pay information can be seen. You will need to re-enter your ID and password (without “ddsb.ca“).  To find the substitute status form, look for `forms online` on the left menu bar. Here, you will be able to access all substitute status forms not yet completed as well as verify forms you have already submitted.   To find the Offense Declaration, click on Employee Services, then click on “Offence”, then click on “Declaration”. 
 
3.If you have Manulife Medi-dent coverage, renewal forms will be mailed to you. Be sure to complete them and return by the requested date if you wish to continue your coverage. Remember, our 12 month `benefit premium holiday` begins July 1st. Your premiums will be waived for the next 12 months.
 
Members who have become newly eligible for benefits (i.e., worked a minimum of 80 instructional days in the elementary panel in the 2015-2016 school year) will also receive forms. If you wish to be included in the benefit plan, be sure to submit your forms by the due date.
